Adult Wellness Visits

Wellness and Prevention are vitally important to your health. I include an annual physical, or wellness visit, in your membership fee because of the high value I place on wellness and prevention. I believe it is important to set aside time once a year to review your overall physical, mental and spiritual health, including aspects such as nutrition, exercise, social situation and life changes, and set a goal for the year ahead. It is also an excellent time to review the preventive guidelines such as recommendations for mammograms, pap smears, prostate screening, colonoscopy, etc. You can review the general recommendations for someone of your gender and age using the Healthfinder.gov program to the right.

The free 60 minute visit is intended for a review of wellness and preventive guidelines only. If you would like to address a new or chronic medical concern in more depth, please schedule additional time for a Follow-Up Appointment click here.

Many people find that they prefer to schedule their Wellness Visit around the time of their birthday. This is a nice reminder to reflect on the previous year and set goals for the year ahead.

1. “How’s Your Health” Survey - click here
2. Medication List: Please bring an updated, detailed list of all medications, supplements, herbs, homeopathics that you are taking. Write down the dosage, how often you take it, and what you take it for. Even better, bring all the bottles!
