Schedule a free introductory visit
- to make sure this practice is the right fit for you. If you have insurance, please check with your insurance company regarding your coverage before our visit. Since I do not contract with any insurance, you will specifically want to know what your contract states about "out of network" providers and:
- Reimbursement for office visits
- Lab tests ordered by an out-of-network doctor (HMOs generally don’t pay)
- Deductibles (individual and family)
If there are still questions, we can discuss it further at our introductory visit.

Print out the Medical History Questionnaire
- Print out the medical history questionaire and fill it out completely! Please note that there are different forms for men, women and children. It should take approximately 1-2 hours to complete the adult forms. All patients should come to the new patient visit with the bottles of all active prescription medications, supplements and vitamins and be prepared to discuss their past medical history in detail. Preparing beforehand by completing the survey will help you get more out of your office visit.

Transfer your medical records
- Each new patient should request a transfer of medical records from their previous physician(s) using the form that is available in the New Patient Packet (it's titled "Authorization to Release Health Care Information.) Feel free to print extra copies to send to additional providers. Inquire into the cost of transferring records before submitting the form! Most practices will transfer records to other doctors for free, but if they want to charge you a lot of money, contact me to decide which elements of the medical record are most essential. I will see you for your first visit even if I have not received this information, but please request the transfer before your appointment.

The Medical History Questionnaire and other registration paperwork should be returned to me at least 3 business days before your new patient appointment, or the appointment will need to be rescheduled. It can be:
- Dropped off in person: If the office is closed, there is a mail slot in the front door. Make sure your papers are in a sealed envelope with my name on it.

- Faxed to: 877-664-6116
- Scanned or e-mailed to: drjess@jessicadavismd.com
