FEE SCHEDULE
This section is an overview of the plans. Please follow the links to get to the details for each plan. These access plans are not health insurance, and only cover services provided by Meliora Family Medicine. Plan A: "Affordable Access" Intended for individuals and families ages 64 and younger Encourages healthier individuals to invest in wellness Better structure for individuals able to get reimbursed by insurance Lower access fee One annual wellness examination is included for no additional charge. Additional office visits or procedures at a reduced fee. Forms can be provided at the end of these visits for patients to submit to their individual insurance plans for possible reimbursement. Plan B: "Balanced Budget" Intended for individuals on Medicare, and individuals with chronic health care issues that require frequent visits with the physician. May also appeal to those that budget their healthcare cost and prefer predictable costs for outpatient medical care. Includes $20 office visits (per 30 minutes). E-mail and scheduled phone consultations are also only $20. One annual wellness exam is included for no additional charge. Click HERE for more details on the fees for Plan B. Plan C: "Customized Charges" Custom made plans available for select individuals who are uninsured or under-insured, and who qualify financially. Please contact Dr. Davis directly regarding availability of scholarships and sliding scale memberships. New Patients Registration Fee: The one time registration fee covers registration and the establishment of care in Meliora Family Medicine. It will also cover your last month of care should you or Dr. Davis terminate this agreement for any reason. The registration fee is paid at the time care is established. The amount is equal to $100 plus 25% of the yearly fee.
