'How's Your Health' Survey 'How's Your Health' is a survey tool that is intended to improve your health and health care. It provides your physician with additional information that helps them pay attention to any gaps in your care. For example: if you are under financial stress, or feel like you do not have enough knowledge about how to manage your diabetes with confidence. It also provides feedback to the doctor about how their practice are serving you, as well as your overall experience with the healthcare system. This information can then be used to make improvements in how the practice operates.
